Welcome to Purpose DNA & Health. By booking a service, using our website, or communicating with us, you agree to the following terms.

Purpose DNA & Health Terms of Service

These Terms of Service apply to appointments and specimen collection services provided by Purpose DNA & Health. Certain services require additional intake forms, consent forms, laboratory documents, or chain-of-custody paperwork.

1. Services Provided

Purpose DNA & Health provides services including:

  • Legal and non-legal DNA testing

  • Non-invasive prenatal paternity testing

  • Relationship DNA testing

  • Mobile and in-office DNA specimen collection

  • Phlebotomy and blood specimen collection

  • Laboratory collection services

  • Collection services for third-party laboratory partners, including Rupa Health, Ulta Lab Tests, My One Medical Source (MOMS), and other approved laboratories

  • Mobile DOT and non-DOT drug screening collections

Purpose DNA & Health primarily acts as a specimen collection and testing coordination provider. Laboratory analysis is performed by independent third-party laboratories unless specifically stated otherwise.

2. Appointments

Services are provided by appointment.

Clients are responsible for selecting the correct appointment type and providing accurate information when scheduling.

Clients should arrive on time for in-office appointments or be available at the agreed location and time for mobile appointments.

Late arrivals may need to be rescheduled if there is not enough time to safely and correctly complete the collection.

Weekend and after-hours appointments are available by appointment only and may require an additional fee depending on the service.

3. DNA Testing Deposits

Deposits apply to DNA testing appointments only.

A non-refundable deposit is required to reserve a DNA testing appointment.

Current DNA appointment deposits are:

  • $35 for standard DNA testing

  • $50 for prenatal DNA testing

  • $45 for mobile DNA testing requests

The applicable deposit is applied toward the DNA testing balance when the appointment is completed.

DNA appointment deposits are non-refundable for missed, canceled, or incomplete appointments except where required by law.

Lab-work appointments do not require a DNA testing deposit.

4. Mobile DNA Services

Mobile DNA testing requires full payment before services begin.

The total may include:

  • DNA testing fees

  • Mobile travel fees

  • Same-day fees

  • Weekend or after-hours fees

Current mobile travel fees are:

  • 0–15 miles: $30

  • 15–30 miles: $50

  • 31+ miles: starting at $75, with the final fee based on location

Same-day appointments may include a $35 fee.

Appointments after 4:00 PM or on weekends require an additional $40 weekend/after-hours fee.

Applicable DNA deposits, mobile travel fees, and surcharges are non-refundable.

If a mobile DNA appointment is canceled at least 24 hours in advance, eligible testing fees, excluding deposits, travel fees, and applicable surcharges, may be considered for a refund at the discretion of Purpose DNA & Health.

5. DNA Appointment Rescheduling

This rescheduling policy applies to DNA testing appointments.

Clients are allowed up to two reschedules without an additional rescheduling fee.

To qualify for a free reschedule, the client must contact Purpose DNA & Health at least 3 hours before the scheduled appointment time.

After two reschedules, a new DNA appointment deposit is required to schedule another appointment.

The new deposit is non-refundable and follows the same deposit requirements as the original appointment.

6. DNA Appointment No-Shows

This policy applies to DNA testing appointments.

A client who does not arrive for a scheduled DNA appointment and does not contact Purpose DNA & Health is considered a no-show.

A no-show will result in:

  • Forfeiture of the original DNA appointment deposit

  • A $15 no-show fee

  • A new DNA appointment deposit being required before another appointment can be scheduled

7. DNA Testing Payments and Refunds

Remaining balances for in-office DNA testing are due according to the requirements of the selected service.

Once a DNA specimen has been collected and submitted for testing, DNA testing fees are final and non-refundable regardless of the test result or personal circumstances, except where required by law.

8. Laboratory and Blood Collection Appointments

Lab-work appointments do not require a DNA testing deposit.

Clients are responsible for selecting the correct laboratory collection appointment and ensuring that required laboratory orders or collection kits are available before the appointment.

Some laboratory partners pay Purpose DNA & Health directly. Clients using those services may have no collection fee due to Purpose DNA & Health.

Other laboratory collection services may require the client to pay a collection fee.

The price and payment requirements shown for the selected appointment apply.

Purpose DNA & Health cannot guarantee that a collection can be completed when an order or collection kit is missing, incomplete, expired, incorrect, or otherwise unsuitable for collection.

9. Rupa Health Collections

Rupa Health appointments may be handled in different ways.

When Purpose DNA & Health creates the client's Rupa Health laboratory order and the collection fee is included through that order, the client does not pay a separate collection fee through Purpose DNA & Health booking.

When another provider refers a Rupa Health client to Purpose DNA & Health and the collection fee is not included in the laboratory order, the client is responsible for the stated $50 collection fee.

Clients should select the Rupa Health appointment type that matches how their laboratory order was created.

10. Partner-Referred Laboratory Collections

Clients referred through approved laboratory partners such as Ulta Lab Tests and My One Medical Source (MOMS) must have the appropriate laboratory order established before collection.

When the laboratory partner pays Purpose DNA & Health directly for the collection, no separate blood draw fee is due from the client to Purpose DNA & Health.

11. Mobile Drug Screening Collections

Purpose DNA & Health provides mobile DOT and non-DOT drug screening collection services.

Drug screening collections are mobile and appointment-based. Specimens are collected according to the requirements of the applicable testing program, employer, laboratory, or referring partner.

Purpose DNA & Health currently provides lab-based specimen collections and does not provide rapid drug testing.

Testing, analysis, and final reporting are performed by the applicable laboratory or authorized testing provider.

12. Specimen Collection and Laboratory Responsibility

Purpose DNA & Health provides specimen collection services and coordinates testing through independent laboratories and laboratory partners.

The performing laboratory is responsible for:

  • Laboratory analysis

  • Testing methodology

  • Laboratory processing

  • Result generation

  • Laboratory reporting

Purpose DNA & Health does not guarantee a particular testing outcome or laboratory turnaround time.

A recollection may be necessary if a laboratory determines that a specimen is insufficient, compromised, rejected, or otherwise unsuitable for testing.

13. Legal DNA Testing

Legal DNA testing requires chain-of-custody procedures.

Clients may be required to:

  • Present valid government-issued photo identification

  • Complete chain-of-custody documentation

  • Allow required identification photographs

  • Follow laboratory-specific identity verification procedures

Failure to follow required procedures may prevent the test from qualifying as a legal chain-of-custody test.

14. Non-Legal DNA Testing

Non-legal DNA testing is intended for personal knowledge and does not follow the same chain-of-custody requirements as legal DNA testing.

Non-legal results generally should not be used when court-admissible or other legally documented testing is required.

15. Prenatal DNA Testing

Prenatal DNA testing has additional eligibility, specimen, identification, payment, consent, and laboratory requirements.

Clients must complete the required prenatal testing acknowledgements and consent documents before collection.

16. Consent Forms, Intake Forms and Identification

Depending on the service selected, clients may be required to complete:

  • Client intake forms

  • DNA testing consent forms

  • Blood specimen collection consent

  • Laboratory order acknowledgements

  • Prenatal testing acknowledgements

  • Chain-of-custody documentation

  • Testing responsibility and results disclosure agreements

  • Identity verification requirements

Booking an appointment does not replace required service-specific consent or laboratory documentation.

Purpose DNA & Health may postpone or decline a collection until required documentation is complete.

17. Client Responsibilities

Clients are responsible for:

  • Providing accurate information

  • Selecting the correct appointment type

  • Providing required identification

  • Following collection and laboratory instructions

  • Having required laboratory orders or kits available

  • Arriving or being available at the scheduled appointment time

  • Completing required intake, consent, and acknowledgement forms

18. Communication

By providing contact information when scheduling an appointment, clients agree to receive service-related communications such as appointment confirmations, reminders, scheduling updates, and information necessary to complete the requested service.

Clients who separately opt in to marketing text messages may receive promotional communications from Purpose DNA & Health.

Message frequency may vary. Message and data rates may apply. Reply STOP to opt out of marketing text messages or HELP for assistance.

19. Privacy and Security

Purpose DNA & Health handles client information according to its Privacy Policy and applicable privacy requirements.

Clients should use the secure methods provided by Purpose DNA & Health when submitting sensitive documents, identification, health information, intake information, or other confidential information.

Additional privacy notices, authorizations, or laboratory policies may apply depending on the service.

20. Collection Safety and Guests

Purpose DNA & Health follows appropriate specimen collection, infection-control, and safety procedures.

Clients are expected to follow collector instructions during the appointment.

Additional guests should be limited unless they are:

  • Being tested

  • Assisting a minor or dependent person

  • Required for the collection

  • Approved in advance

21. Limitation of Responsibility

Purpose DNA & Health is not the laboratory performing analytical testing unless specifically stated otherwise.

Independent laboratories are responsible for specimen analysis and laboratory reports.

Purpose DNA & Health does not provide medical diagnosis or treatment and does not interpret laboratory results as a substitute for advice from a licensed healthcare provider.

Purpose DNA & Health is not responsible for decisions made by clients or other parties based on test results.

22. Agreement to Terms

By booking an appointment and proceeding with services, clients acknowledge these Terms of Service and agree to the payment, scheduling, collection, identification, consent, and service-specific requirements that apply to their selected service.

Service-specific signed consent forms, acknowledgements, laboratory documents, or chain-of-custody requirements may contain additional terms applicable to that particular service.

23. Changes to These Terms

Purpose DNA & Health may update these Terms of Service as services, policies, laboratory partnerships, or business requirements change.

The current version will be posted on this page.
