1. Services Provided

Purpose DNA & Health provides:

• DNA testing services (legal and non-legal)

• Prenatal DNA testing (non-invasive)

• Mobile and in-office phlebotomy services

• Lab specimen collection services

• Assistance with third-party lab testing (Ulta Lab Tests, Rupa Health, and others)

We are a collection service provider. Laboratory testing is performed by third-party labs.

2. Appointments

All services are by appointment only.

• Appointment times are scheduled based on availability

• Clients must arrive on time or be available at the scheduled mobile appointment time

3. Deposits & Payments

A deposit is required to secure your appointment.

• Deposits are non-refundable

• Deposits go toward the total cost of your service

Remaining balances are due:

• At the time of service, or

• As outlined during booking

Payments are processed through Stripe via Carepatron or other approved systems.

5. No-Show Policy

Failure to show up for your appointment without notice will result in:

• Loss of deposit

• Requirement of a new deposit to reschedule

6. Mobile Service Fees

Mobile appointments may include additional fees such as:

• Travel fees

• After-hours or weekend fees

These fees will be disclosed at the time of booking.

7. DNA Testing Terms

Legal DNA Testing

• Requires valid government-issued identification

• Follows strict chain-of-custody procedures

• Results may be used for legal purposes

Non-Legal DNA Testing

• For personal knowledge only

• Cannot be used in court

Prenatal DNA Testing

• Requires additional consent

• Must follow lab-specific requirements

Purpose DNA & Health is not responsible for laboratory processing times or results.

8. Lab Testing Services

We assist clients with lab specimen collection.

• Lab tests are processed by third-party laboratories

• Clients may order their own labs through platforms such as Ulta Lab Tests or Rupa Health

• We are not responsible for lab results or delays

9. Client Responsibilities

Clients agree to:

• Provide accurate information

• Bring required identification (for legal testing)

• Follow pre-test instructions (ex: no eating before certain tests)

• Be available and prepared at appointment time

10. Communication Consent

By providing your contact information, you agree to receive:

• Calls and messages regarding your appointment

• Service-related updates and reminders

11. Refund Policy

• Deposits are non-refundable

• Testing and service fees may be non-refundable once services are performed

• Any exceptions are at the discretion of Purpose DNA & Health

12. Limitation of Liability

Purpose DNA & Health is not liable for:

• Lab processing delays

• Lab errors or results

• Misuse of test results

• Decisions made based on test results

All services are provided “as is” without guarantees.
