Policies and Procedures
Appointment & Payment Policy
To ensure smooth scheduling, the following payment terms apply:
Non-Refundable Appointment Deposits
A $25 non-refundable deposit is required to schedule most DNA testing appointments.
A $50 non-refundable deposit is required for Prenatal Paternity Testing.
This deposit secures your appointment and is applied toward your total balance if the appointment is completed.
If the appointment is canceled, missed, or incomplete, the deposit is non-refundable.
Mobile DNA Testing Fees
Mobile service includes travel, time, and convenience fees. These fees are in addition to your DNA testing cost and must be paid in advance to confirm your appointment.

Distance Travel Fee (30+ miles) | $100.00 + $1.00 per mile over 30 miles | Applies to any location 30+ miles away |
Same-Day Appointment Surcharge | $35.00 | Applies to appointments scheduled same day |
Evening/Weekend Surcharge | $40.00 | For appointments after 5:00 PM or on weekends |
Prenatal Paternity Mobile Fee | $175.00 | Flat rate for appointments within 50 miles |
Note: If a mobile appointment is prepaid and canceled at least 24 hours in advance, testing fees (excluding deposit and travel surcharges) may be refunded. All deposits are non-refundable.
DNA Testing is Non-Refundable
Once a sample is collected, the full cost of testing is non-refundable, regardless of test type.
This applies to both legal and non-legal testing services.
In-Office Testing Payment Policy
No payment is collected until you arrive for your appointment.
A deposit is still required to book the time slot, and it is non-refundable if canceled or missed.
Legal Testing Requirements
A valid government-issued photo ID (e.g., license or passport) must be uploaded to the Carepatron portal or presented in person.
A live photo will be taken at your appointment for legal documentation and verification.
Using the Carepatron Portal
To remain compliant with HIPAA and privacy regulations, clients must:
Complete the Intake Form
Sign the Consent to Test & Services
Upload any required identification
Agree to these Policies & Procedures
Access all results and communication securely through Carepatron
All data is stored in encrypted, HIPAA-compliant systems.
Appointments, Rescheduling & Arrival
All appointments require a deposit for confirmation.
Any applicable mobile service fees must be paid in advance.
You must provide at least 24 hours’ notice to reschedule without penalty.
No-shows or missed appointments forfeit the deposit and any mobile service fees.
If you're running late, please call us. Collectors may wait up to 30 minutes before proceeding to their next appointment.
Collector Identification & Safety
All collectors wear a company badge or name tag for verification.
You’ll receive a confirmation text or portal message with the collector’s name before your appointment.
All equipment is sanitized, and hygiene practices are followed at every visit.
Note: Please do not bring additional guests unless they are also being tested.
Collection Locations
Mobile DNA collections occur in safe, public locations (parking lots, community centers, etc.).
Collectors do not enter private homes under any circumstances.
Detailed instructions are provided via text or Carepatron before your appointment.

The laboratory performing the analysis is an independent entity responsible for testing, processing, and issuing results.
By using our services, you agree that Purpose DNA and Health is not liable for test outcomes, the actions taken by any party based on those results, or any claims, disputes, or legal actions that may arise.
