2. Appointment Is Scheduled

Your collection appointment is scheduled through the My One Medical Source (MOMS) platform.

3. Bring Your Kit

If a collection kit was mailed to you, please bring the full kit and instructions to your appointment.

4. Professional Collection Completed

Purpose DNA & Health completes your specimen collection in a private and professional setting.
